ACTS Foundation provides no-cost Enhanced Care Management for eligible Medi-Cal members in Fresno, Madera, Tulare, Kings, and Orange counties. A dedicated care manager helps coordinate healthcare, behavioral health, housing, transportation, food assistance, and other community support.
Available through participating Medi-Cal health plans. Orange County services are currently available to Kaiser Medi-Cal members only. Anthem members must be age 20 or younger.
