What is CalAIM?

CalAIM is a new approach by the State of California to transform the Medi-Cal system and provide whole-person care. It’s about more than doctor visits — CalAIM connects you with services that support your health, housing, and well-being.

What is Enhanced Care Management (ECM)?

ECM provides a personal care team that works with you to:

Coordinate your medical, behavioral, and social services
Support you through life transitions such as hospital discharges or homelessness
Connect you to housing, food, transportation, and community resources
Help you create a personalized care plan and stick to it

You’ll have a dedicated care manager who checks in regularly and advocates for your needs.
