Free Enhanced Care Management for Medi-Cal Patients
The ACTS Foundation is proud to now offer CalAIM Enhanced Care Management (ECM) services—at no cost to eligible Medi-Cal recipients.
CalAIM (California Advancing and Innovating Medi-Cal) is a statewide initiative designed to improve health outcomes for individuals with complex needs.

What is CalAIM?
CalAIM is a new approach by the State of California to transform the Medi-Cal system and provide whole-person care. It’s about more than doctor visits—CalAIM connects you with services that support your health, housing, and well-being.
What is Enhanced Care Management (ECM)?
ECM provides a personal care team that works with you to:
Coordinate your medical, behavioral, and social services
Support you through life transitions (such as hospital discharges or homelessness)Connect you to housing, food, transportation, and community resources
Help you create a personalized care plan and stick to it
You’ll have a dedicated care manager who checks in regularly and advocates for your needs.
Who Can Benefit?
ECM is designed for Medi-Cal recipients who:
Are experiencing homelessness or at risk of losing housing
Have complex physical or behavioral health conditions
Have frequent hospitalizations or emergency room visits
Are transitioning out of jail, foster care, or skilled nursing facilities
Live with serious mental illness or substance use disorders
Are pregnant and facing barriers to care
